SUBMISSION OF MAKE UP EXAMINATION FORMS

It is notified for the information of all under-graduates students of Admitted in Fall (2012-2016) and Post-graduates students of Fall (2013-2015) of IBMS and Affiliated Institutes to submit their Make up Examination Forms for their failed papers of 2nd,4th,6th & 8th Semester if any as well as 2nd Semester of Post graduates students as per following Schedule:

S.#	Schedule	Submission
1	Normal fee	Up to 21th April, 2017
2	with late fee of Rs. 500/-	Up to 28th April, 2017

The concerned students are directed to submit their forms to sectional heads in IBMS while the students of Affiliated Institutes to incharge of their Institute.

Note:

No form will be entertained after due date i.e. 28th April, 2017.
Attach Two latest attested photographs with the examination form.
Attach attested DMC showing failed subjects of the semester applied for.
Submit separate Exam form for each semester along with separate Bank receipt.
Attach original receipt of Rs.1000/- Per Subject.
